This report, published in May 2010, argues that the
Personalisation model is particularly notable for offering a
framework for integrating health and social care support to people
with mental health problems. It draws on the latest thinking and is
designed to promote personalisation, empowerment and
citizenship.
The report focuses on a model for development and implementation
in Yorkshire & Humber but also provides a framework for the
integration of NHS and social resource allocation systems.
