Meeting the Challenges of Personal Health Budgets

Does your CCG need some practical help and advice to support the further roll-out of personal health budgets? The Government's Mandate expects 50,000 -100,000 personal health budgets (PHBs) by 2020/21, as well as CCG responsibilities regarding the legal right to have a personal health budget for people who are eligible for NHS Continuing Healthcare and continuing care for children. The Next Steps on the Five Year Forward View published in March 2017 further promotes supporting disabled people and people with complex health needs through personal health budgets, with plans to reach in excess of 40,000 people by 2019.

In Control have been helping Commissioners within Clinical Commissioning Groups (CCGs) who are responsible for ensuring that the governance (financial, safeguarding and clinical aspects) of PHBs are proportionate and that all budgets are spent in demonstrable ways of meeting the outcomes of the agreed personal care and support plans. This has been both strategic and individually. Julie Stansfield from In Control has worked with a range of CCGs to devise an internal pathway policy for Personal Health Budgets. This has included becoming a critical friend to the PHB lead and operational group and supporting with the sticky issues that occur as people are working their way through the process for both children and adults. Julie has also played the role of independent representative in circumstances of complexity, complaint or multi-agency involvement.

    PLEASE ASSIST IF POSSIBLE WITH MORE INFORMATION AS TO LEVELS OF INTERVENTION WHICH COULD BE REQUESTED TO COMPLETE PERSONAL SUPPORT PLAN FOR MY MUM AND FAMILY .
    URGENT ACTIONING REQUIRED FOR THE NOMINAL PERSONAL HEALTH BUDGET ,AS AWARDED,AT THIS TIME ,AS FACING REPERCUSSIONS FROM CCG STAFF OVER ONGOING CARE PROVIDER ORGANISATIONAL /STAFFING ISSUES,REGARDLESS OF RECENT FORMAL COMPLAINT VIA SOCIAL SERVICES.
    STRAINED RELATIONS AND CONFLICT OF INTERESTS ARE BEING PERPETUATED BY CCG WITH CURRENT CARE PROVIDER AND POOR LACK OF TRANSPARENCY IS CONSISTENTLY LOW.
    MUM/FAMILY NOW IN RECEIPT OF" SUBJECT ACCESS" ASSESSMENT DOCUMENTS PREVIOUSLY NOT DISCLOSED WITH FAMILY. WHICH SEEM BASED ON OUTDATED REVIEW DETAILS,POSSIBLY APRIL 2017 ,RATHER THAN NOVEMBER 2017 PRE BUDGET REQUEST REVIEW MEETING.
    SPECIALIST ADVOCACY IS UNAVAILABLE SO IS APPROPRIATE CULTURAL SERVICES LACKING , HENCE ALTERNATIVE SERVICES NOW SOURCED BY FAMILY TO ORGANISE QUALITY WHOLISTIC SERVICES AND
    ALTERNATIVE SPECIALIST PROFESSIONALS .
    ALTERNATIVE APPROPRIATE DOMICILIARY/MULTIPLE RESPITE SERVICE
    IS LACKING,WHICH IS UNACCEPTABLE AND NOW 18 MONTHS OF INCREASED DISTRESSING END-OF-LIFE/COMPLEX HEALTH CONDITIONS. WHICH INCIDENTALLY CCG ATTEMPTED TO REVOKE FUNDING FOR PRESENT SERVICE FUNDING AS FAR BACK AS OCTOBER 2017.
    INDIFFERENCE TO WAIT PATIENTLY FOR PERSONAL HEALTH BUDGET HAS NOW BECOME VERY DISTANT ,PARTICULARLY AS PERSONAL BROKERAGE COMPANY WERE REFUSED AUTHORISATION TO ASSIST UNTIL NOMINAL BUDGET ACCEPTED.
    MOTHER HAS MENTAL LACK OF CAPACITY AND FAMILY HAVING POWER OF ATTORNEY WHICH AGAIN BEEN DISMISSED AS IRRELEVANT AS MUCH AS POSSIBLE,IN VARIOUS INSTANCES AND ISSUES.
    PRESENT TIME HAVE SOURCED LOCAL MANCHESTER HEALTH FORUMS IN RECENT MONTHS ,TO EMBRACE INFORMATION ON APPROPRIATE SERVICES INCLUSIVE PERSONAL INTEGRATED HEALTH BUDGETS.
    SOME LOCAL BOLTON AND BURY CCG LEAD PROFESSIONALS WERE PRESENT.
    THESE FORUMS ALSO ATTENDED BY SOME CHIEF OFFICERS WHOM I MANAGED TO SHARE VARIOUS DISCUSSIONS WITH ,WHILST UPDATING OVER ALTERNATIVE CURRENT PRACTICES AND SERVICES.
    LASTLY THANKS FOR YOUR TIME AND ANYMORE PROGRESS WOULD BE APPRECIATED TO RESOLVE SUPPORT ISSUES AND BE CLOSER TO QUALITY LIFE OUTCOMES.
